CCO Performance Report Feedback Form - 8 Nov 2011
The following is the text of an email sent by the Critical Contingency Operator to industry participants on 8 November 2011.
“Industry Participants,
Following the recent Maui pipeline outage the CCO published an incident report on 4 November 2011 as required by r64. The CCO is also required to prepare and publish a performance report as required by r65. The purpose of this report is to evaluate how well the contingency arrangements performed during the critical contingency. It assesses the performance of the CCO and transmission system owners during the contingency and it evaluates the effectiveness of the critical contingency management plans, the communications plan, and the information guide and the extent to which the arrangements achieve the purpose of the regulations. The report is also required to identify, where applicable, any amendments to the regulations or other arrangements that would better achieve the purpose of the regulations.
Because of the scale and duration of the incident, the CCO would like to include the experiences of as wide a range of stakeholders as possible in the assessment of how the contingency arrangements operated during the contingency. Your comments and feedback on the critical contingency are now invited for inclusion in the performance report. To assist this process a performance report feedback form has been developed and is attached. If you wish to comment please send responses by email to cco
vector [dot] co [dot] nz or by telephone at 06 759 6525. The closing date for comments is Friday 18 November 2011.
Your comments will be used by the CCO to prepare a draft performance report which will be made available for public comment prior to a finalisation. It is planned to publish the draft performance report on the CCO and Gas Industry Company (GIC) websites by Friday 25 November 2011 and five business days will be provided for comment. It should be noted that if there are aspects of your submission that are confidential, please note them as such. Confidential information used in preparing the report will not be attributed to its source.
For those retailers receiving this email you are invited to communicate details of this process to your consumers affected by the outage and to encourage them to provide feedback.
Copies of the CCO incident report and the feedback form can be obtained from the publication section of the CCO website at http://www.oatis.co.nz and on the Gas Industry Company website at http://gasindustry.co.nz.

Critical Contingency Incident Report - 4 Nov 2011

I can now advise that the CCO has published an Incident Report regarding the Maui pipeline outage that occurred between 25 October and 30 October 2001 in accordance with the requirements of r64 of GG(CCM)R 2008. A copy of the report can be obtained from the publications section of the CCO website at www.oatis.co.nz.
The CCO is also required to prepare and publish a Performance Report in accordance with the requirements of r65. The CCO is currently establishing a consultation and feedback process to inform the contents of this report. Because of the scale and duration of the event, the CCO would like to include the experiences of as wide a range of stakeholders as possible in the assessment of how the contingency arrangements operated during the contingency. Details of the feedback and consultation process will be released early next week.

Gas Industry Co has granted an extension from 20 to 30 business days for the CCO to publish the Performance Report, recognising the extent of the event, the number of people affected and the desire to ensure full input into the CCO’s consultation process.
